Incident Investigation Recommendations

At location AFC 105-C3 top platform, a spark occurred on an electrical extension board during work. No injury or damage was reported. During the investigation, it was identified that the extension board’s wire was cut due to contact with a metallic object, which caused the spark. The faulty extension board was removed from the location.

Avoid placing extension boards/wires in direct contact with metallic objects or sharp edges.
