Description
At tank # 56, during fit up activity of 3rd shell course, the tack weld of the U-fit up accessories broke as a result of hammering and fell down in the barricaded area. No injury or damage occurred.
Recommendations
Ensure all other installed U-clamps are inspected by the QC inspector to confirm the integrity and quality of their welding joints.
