Description
At tank # 56, during fit up activity of 3rd shell course, the tack weld of the U-fit up accessories broke as a result of hammering and fell down in the barricaded area. No injury or damage occurred.
Recommendations
All U-clamps should have secondary retention by securing chains welded to the wedges.
