Description
At tank # 56, during fit up activity of 3rd shell course, the tack weld of the U-fit up accessories broke as a result of hammering and fell down in the barricaded area. No injury or damage occurred.

Metallic mesh to be installed in the areas below work at height to safely arrest any falling objects
