Incident Investigation Recommendations

At Rotary workshop, a sudden leakage from top hose assembly of 5 kg CO2 fire extinguisher occurred, resulting in high pressure discharge with an audible release from the cylinder. Due to leakage a safety sign board positioned above also fell. No injury was reported.

Fire team will visit the vendor site to witness the hydrotest activity.
