Description
During major Repair project 2024-25 by M/s IIW, while the positioning of the 25 ton crane for cutting of Tank-04, it made contact with the tank scaffolding. Consequently the scaffolding pipe was bent as a result of this incident.
Recommendations
Update the Job Safety Analysis (JSA) for crane positioning to include a safe exit procedure from the crane after it has been positioned.
