Description
At tank # 56, channel of length approx 1.5ft fall from the 4th shell course from outisde of tank. The incident was occur during fitup activity of wind girder.
Recommendations
Conduct a Toolbox Talk (TBT) on Dropped Object Prevention for personnel involved in work-at-height activities.
