Incident Investigation Recommendations

At 0840 hrs., A supervisor was taking a water sample from water softening (cation unit # 2) sampling point using a glass bottle. After collecting the sample, he attempted to stand up, but his foot twisted, causing him to lose balance and fall to the ground. As a result, the glass sample bottle he was carrying in his right hand broke, and he sustained minor cuts to his right thumb and near the palm.

Use protective rubber, plastic, or leather carriers/covers for glass sample bottles.
