Observation

At Tank #11, a loaded boom truck developed a mechanical fault; the clutch and gear system were not functioning properly. The HSE Lead and Project CM were informed. The boom truck was immediately stopped and isolated, and the operator was instructed to park the vehicle in a safe area. The issue was reported to the mechanical/maintenance team for inspection and repair. The boom truck will remain out of service until the fault is rectified and verified as safe for operation.
